Cascade Hemophilia Consortium ("Cascade") provides patients and their caregivers the ability to communicate with us through an encrypted, HIPAA compliant, secure webform (texting). To activate this service, we will send you and anyone you designate an SMS text message to confirm your sign-up. This will come from a 323 area code phone number, and the text messages you will receive will always come from the same number.
You can cancel the SMS texting service at any time by texting "STOP" on the text message communication, and we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.
If at any time you forget what keywords are supported just text "HELP" on the text message communication and we will respond with instructions on how to use this service as well as how to unsubscribe.
We are able to deliver messages to the following major mobile phone carriers: AT&T, Verizon Wireless, Sprint, T-Mobile, MetroPCS, U.S. Cellular, Alltel, Boost Mobile, Nextel, and Virgin Mobile. We also are able to deliver messages to a number of regional carriers. Please check with us in advance if you are not using one of the major carriers. We do not guarantee that we will be able to deliver messages through any mobile phone carrier and we are not liable or responsible for any messages that are not received, delays in receipt, or mobile phone carrier, software, hardware, network, or other issues.
Please be aware that this secure messaging platform should not be used for any emergency medical situations, and you should always call 911 and seek immediate medical attention in case of an emergency. If at any time you have an urgent question, do not understand a message we send you or do not feel comfortable discussing an issue via text message, you should call Cascade instead of communicating with us via text message. You agree that Cascade will not be responsible for providing prompt or timely responses to your text messages and that Cascade does not regularly check text messages that you may send to Cascade at any standard frequency or outside of normal business hours (including, without limitation, nights, weekends, and holidays). Please call Cascade at 734-996-3300 to confirm that your text message was received if you haven't received a response by text message or telephone within a few hours.
Risks of using text messages to send protected health information include, but are not limited, to:
Risk of Unauthorized Access by a 3rd Party: Do you share a phone or other devices with your family? Is your phone or access to phone provided through your employer? Do you access your text messages over an unsecured connection such as public Wi-Fi? Do you access your text messages on your mobile device? Text messages may be accessed by someone you do not wish to know about your health information. Despite necessary precautions, text messages may be sent to the wrong address by either party. Text messages may be intercepted or altered in transmission by a computer hacker or computer virus.
Unique Difficulty in Verifying the Sender: Text messages may be easier to forge than handwritten or signed papers. Cascade will only send text messages to the cellular phone number that you provide, but it may be difficult to confirm that you are in fact the person sending the request for information via text message on your phone.
Please also note that neither we nor the service providers associated with the secure webform are liable for delayed or undelivered messages, or for the security of the messages on your or your caregivers' personal devices. We also are not responsible for any incorrect information provided to us, including incorrect phone numbers, and you should notify us at once and in writing if you or anyone you have authorized to receive messages on your behalf change phone numbers, loses a phone or other personal device, suspects that the security of a phone or other personal device has been compromised in any way, or you wish to add or delete someone authorized to receive messages on your behalf.
As always, message and data rates may apply for any messages sent between us. You will receive a new message each time we communicate with you through the secure messaging platform. If you have any questions about your text plan or data plan, you must contact your wireless provider.
By signing below, you acknowledge your recognition and understanding of the inherent risks of communicating your health information via text message and hereby consent to receive such communications via text message despite those risks. Text messages containing clinically relevant information may be incorporated into the medical record at Cascade's discretion.
By signing below, you also acknowledge that you have the choice to receive communications via other means such as by telephone. By signing below, you agree to hold Cascade harmless for unauthorized use, disclosure, or access of your protected health information sent to the cell phone (via text message) you provide.
